Cancellation of
appointment
Please kindly notify us 24 hours in
advance if you cannot keep your
appointment. If 24 hours notice is not
possible, please call us as soon as
possible. There will be a missed
appointment fee if you fail to do so.
There will also be termination from our
practice if there are more than 3 missed
appointments without notification.
Calls during office hours
Emergency or urgent calls will be
returned right away or immediate
appointments will be offered. Dr. Maw
will return the non-urgent calls between
patients or at the end of the day.
After Office hours, weekends,
holidays
Please call 911 if you have a life
threatening emergency. If you have an
urgent matter to talk to the doctor
about, please call our office number and
the answering service will page Dr. Maw.
If you do not receive a call back,
please do not hesitate to call again.
Payment
Copayment and balance are due at the
time of visit. Copayment is required by
your insurance for each visit including
follow up visits for the same condition.
The deductibles and the balance
indicated by your insurance is your
responsibility. It will be billed and
payment is expected within 60 days. We
accept cash, check, Visa, Master,
Discover and Debit Card. If there is no
attempt to make any payments for more
than 90 days, the account will be sent
to the collection agency. If the account
is seriously delinquent, we may not be
able to provide medical care for your
child/ children anymore.

Prescriptions
Antibiotics or any new medication for
the child will not be prescribed over
the phone without being seen in the
office. Please call the pharmacy for a
medication refill for chronic
conditions. The pharmacy will send the
request electronically. It will not be
refilled if the child has not been seen
at the office within 6 months, in which
case an appointment is needed.
